Be sure you have at least version 8 of Adobe Reader. If not, you can download the latest version by clicking the link below and following the directions.
In order to submit the application by e-mail, it's best to have your e-mal application open when you're ready to submit. Web-mail applications like Hotmail, Yahoo, GMail, AOL aren't compatible with this feature of Adobe Reader.
If you don't have an e-mail application on your computer or receive your e-mail through Hotmail, Yahoo, GMail, AOL or other on-line source, you will have to print the form after filling it out and mail it or drop it by our office or fill out our on-line application. Please remember that you can not save the form once you've filled it out. This is a limitation of Adobe Reader.
Once you have completed the application, press the "Print Form" button on the last page to print a copy for your records. Then press the "E-mail Form" button on the last page to e-mail the application to us. We look forward to hearing from you.
